To: Provident Oil and Gas Company whose last known address is 1653 Merriman Road, Akron, Ohio 44313. Certified mail to that address was undeliverable. To: A.J. Lowe for whom no last known address cannot be located after a diligent search. To: John M. Van Vliet for whom no last known address cannot be located after a diligent search. Lease: O.J. and P. L. Baumgardner to Park Ohio Industries, Inc., recorded May 12, 1977 covering 58 acres described by bounding landowners in Section 29 and 10 in Richfield Township, Summit County, Ohio, Volume 5845, Page 324. Lease: Ora J. and Virginia M. Baumgardner to Provident Oil and Gas Company, recorded June 17, 1983 covering 25.83 acres described by bounding landowners in Sections 9 and 10 in Richfield Township, Summit County, Ohio, Volume 6731, Page 717. Lease: Paul L. Baumgardner to Provident Oil and Gas Company, recorded June 17, 1983 covering 24.91 acres described by bounding landowners in Section 10 in Richfield Township, Summit County, Ohio, Volume 6731, Page 725. Lease: Scott Baumgardner and Belle Baumgardner, his wife, to A.J. Lowe, recorded August 22, 1917 covering 74 acres described by bounding landowners in Summit County, Ohio Book 538, Page 486. Lease: Scott Baumgardner to John M. Van Vliet, recorded August 14, 1907 covering 108 acres described by bounding landowners in Richfield Township, Summit County, Ohio Book 307, Page 464. Description of Land as to all Leases: The lands owned by successor Lessor covers 52.6862 acres of land, more or less situated in Lots 9-10, Tract 3, Richfield Township, Summit County, Ohio being parcel number 48-02007 which real estate was conveyed to Monarch Field, LLC, an Ohio limited liability company by Limited Warranty Deed dated October 19, 2019 and recorded October 21, 2019 as Instrument Number 56500382 in the Summit County Fiscal Office land records. Cause of Forfeiture as to all Leases: Pursuant to the terms of the leases, the leases expired during their primary term in which no well was drilled on the property during the primary term of the leases to extend it into its secondary term. These leases are now entitled to cancellation. It is the intention of the current property owner, Monarch Field LLC, to file for record, an Affidavit of Forfeiture, with the Summit County Fiscal Office in Summit County, Ohio, declaring these leases forfeited if you, or the assignee of said lease above referenced, do not have the lease released of record within thirty (30) days from the date of this publication.
